When Outpatient Therapy Isn't Enough Anymore

Most people who come to Thira have already tried. They've done the weekly therapy. They've tried medication. Maybe they've been to a program before that didn't stick. And things are not improving.

You might be here because:

  • Depression has stopped feeling like something that comes and goes — it's just there now
  • Anxiety or panic is running the day, not you
  • Food, body image, or eating has become its own full-time problem
  • Trauma symptoms are louder than they used to be — flashbacks, avoidance, numbness
  • Self-harm or suicidal thoughts are showing up more often
  • A teen daughter is in crisis and school counselors, ER visits, or outpatient therapy haven't been enough

What Makes Thira Different

DBT is one of the most-researched, most-effective treatments for emotional dysregulation, self-harm, suicidality, and borderline personality. But most programs that claim to offer DBT actually run only one or two of its components. We run all four — individual therapy, skills training groups, between-session coaching, and a consultation team for our therapists. Our Chief Clinical Officer, Dr. Kathryn Korslund, trained directly under Dr. Marsha Linehan — the creator of DBT — at the University of Washington.

True DBT

Healing is different when you're not the only person in the room who's scared to talk about what actually happened to you. Every level of care at Thira (except Virtual IOP) is designed specifically for women, teen girls, and gender non-conforming people. It's not a co-ed program with a women's track — it's the whole program.

Thira grew out of The Moore Center, founded in 1991 by Dr. Mehri Moore, the longest-established women's mental health treatment center in the Pacific Northwest. Today, we have over 100 clinicians: psychiatrists, licensed therapists, HAES-informed dietitians, expressive arts specialists, nurses, and behavioral health techs.

What We Treat

We work with the full range of mental health conditions that bring women and girls into a higher level of care:

  • Depression — including persistent, treatment-resistant, and post-partum
  • Anxiety disorders — generalized, social, panic, and phobia-related
  • Trauma and PTSD — including complex trauma
  • Bipolar and mood disorders
  • Disordered eating, body image, and co-occurring eating concerns
  • Self-harm and suicidal ideation
  • Emotional dysregulation and interpersonal difficulties
  • Co-occurring mental health conditions
  • Thira treats mental health only — we are not a substance use / addiction treatment program

The People Behind the Program

Dr. Mehri Moore - Founder

Board-Certified Psychiatrist. Life Fellow of the American Psychiatric Association. Dr. Moore has spent 35+ years working specifically with women and girls in mental health. In 1991 she founded The Moore Center — the longest-established women's mental health treatment center in the Pacific Northwest — which became Thira. She trained at the University of Washington and Philadelphia Child Guidance Clinic (CHOP / University of Pennsylvania).

David Lim, LICSW, MSW, MBASenior Clinical Director

David holds fidelity-level training in three of the most evidence-based modalities in mental health treatment — DBT (Behavioral Tech), EMDR (EMDR International Association), and IFS (IFS Institute, Level 2). He has led clinical teams at Fairfax Hospital, Atlantic Street Center, and NAVOs, and taught graduate coursework in Motivational Interviewing, Psychopharmacology, and Psychosocial Assessment at Bastyr University, Seattle University, and USC. At Thira, he builds clinical cultures grounded in interdisciplinary collaboration, autonomy, and measurable outcomes.

Patient Stories

Eve K.

Thanks to THIRA, I know how to stay calm instead of exploding during hard conversations. I know how to take a step back to observe so I can use my wise mind. I know how to not take every single thing anyone says super personally. But the most helpful thing (a true godsend) is I now know how to set boundaries.

Lorelai S.

Overall the staff at Thira are awesome. You can tell they truly care about their jobs and want to help us. And yesterday I left Thira willing and sad about leaving. I didn’t think an 8 week (6-8 weeks, but I did the 8) program would change me so much, and it truly did.

Abbey R.

As an outpatient eating disorders dietitian, I have seen at least 10 clients after their discharge from THIRA over the last 1 year. Clients have all expressed positive experiences where they felt heard, cared for, and respected. THIRA continues to learn and practice Health At Every Size and use weight-neutral language at their center - something I believe to be vital for recovery and inclusion.

Three steps. No long intake maze.

Step 1: Request a Free Assessment

How It Works

Call or fill out the form on this page. Takes about two minutes.

Step 2: Talk to a Real Clinician

A member of our admissions team — not a call center — will call you back. We'll talk through what's going on, verify your insurance benefits, and help figure out which level of care is the right fit.

Step 3: Start Treatment

If Thira is a fit, we'll walk you through the next steps and scheduling. If we're not the right fit, we'll tell you and help point you in the right direction.

No cost. No obligation. Fully confidential.
In the assessment we'll:

Your Free, Confidential Assessment

Your information is protected. We never share your personal or health information without your consent. By submitting this form, you consent to being contacted by THIRA Health about treatment options. Privacy policy.

  • Listen to what's actually going on — no rushing
  • Verify your insurance benefits so you know what's covered
  • Recommend the level of care that fits — Residential, PHP, IOP, or Virtual IOP
  • Answer any question you have about DBT, our program, or what treatment actually looks like day-to-day

If Thira isn't the right fit, we'll say so.

Will my insurance cover this?

We're in-network with most major commercial plans in Washington — Premera, Regence, BCBS, Cigna, Optum, UHC, Kaiser, Anthem, First Choice, Asuris Northwest, Lifewise, Carelon, and Multiplan. Aetna and WA Medicaid are case-by-case. We'll verify your specific benefits during the free assessment so there are no surprises.

What's the difference between Residential, PHP, IOP, and Virtual IOP?

Residential is 24/7 — you live at Thira. PHP is full clinical days with evenings off. IOP is several sessions a week around your schedule. Virtual IOP is the same IOP clinical model delivered fully online. Most people aren't sure which one they need before they call — that's part of what the assessment is for.

Is Thira only for women?

Our Residential, PHP, and in-person IOP programs are designed for women, teen girls, and gender non-conforming people. Our Virtual IOP program is open to all genders.

Is this evidence-based, or more of a wellness retreat?

Evidence-based. Thira runs true adherent DBT — all four components — with a clinical team of 100+ including psychiatrists and licensed therapists, and Joint Commission Gold Seal accreditation. Our CCO trained directly under the creator of DBT.

What if I'm not sure I'm "bad enough" to need this?

Most people we talk to feel that way. That's why the assessment is free. Call and tell us what's going on — we'll help you figure out whether Thira is the right level of care, whether something lower-intensity would serve you better, or whether you should be looking elsewhere.

Do you treat teens?

Yes. We have dedicated residential and IOP tracks for adolescent girls (ages 13–17) and a summer immersive program. Parents and guardians handle the initial call — we'll walk you through everything.
